Survey to evaluate End of Life Doula services

As part of a HEIF KTV funded project with End of Life Doula UK (EoLDUK) members of Open Thanatology developed a survey to help EoLDUK evaluate their services. This file is the survey questions. 

The questionnaire included questions relating to quality of care, key performance indicators aligned with the commissioning arrangement, and questions aligned with other quality of end-of-life questionnaires, in particular 'Care Of the Dying Evaluation' (CODETM). Permission was gained from the authors of CODETM to utilise appropriate CODETM questions within the survey. There was a mixture of closed and open questions, with 18 questions in total. The survey was designed to be completed online or on paper. Prior to launching the questionnaire, it was peer reviewed to ensure readability and sensitivity thus ensuring face validity.
